It is VHA policy that veterans with chronic medical conditions are eligible for a personalized medical ID bracelet, pendant or dog tag through the Prosthetics and Sensory Aids Department. The medical ID is no cost to the veteran and must be placed via an electronic consult to Prosthetics by a VA Clinician or other approved health care provider.
Veterans with the following chronic medical conditions are candidates for a medical ID:
- Diabetes
- Heart Conditions
- Food or Drug Allergies
- Substance Use Disorders
- Dementia
- Embedded Metal Fragments
- Seizure Disorder
- Anticoagulation Therapy
- Pulmonary Diseases
- Rare Diseases
