I need to order prosthetic stump socks from VA ASAP. Which form do I use?

Answer:

Prosthetic socks are available from the VA Denver Acquisition and Logistics Center (DALC). DALC maintains an inventory of standard-sized prosthetic socks which are available for immediate delivery. If the specific prosthetic socks you need are not stocked at the DALC, VA will order them from the vendor. When they are received, they will be sent directly to you.

You may place orders with the DALC at least 30 days before you need the ordered item. To order, you may:

1.      Complete VA Form 2345, Veteran’s Request for Prosthetic Socks and mail it to the DALC.

2.      Send an order via email to dalc.css@va.gov.
